I'll 2nd hatred on bunny hoppers. Thankfully most games curb that on their own now by making the jumps real short
MMO's are such fluid/changing games that it's incredibly hard to say. For example, I think SO differently of "Cataclysm WoW" than I do of "vanilla WoW". But let me give this a try....
FYI, I am not including SWTOR in this list. Too early to make a big judgement like that (but it would likely be top, or at least in the top 3 right now for sure).

I think the game that made me rage the most was probably Mario Kart. It's also one of the top 3 games for me all-time, so I don't want you all to think I didn't like it. But the built-in comeback mechanics where the player in last place all of a sudden started getting triple-red-shells, stars, and all types of awesome stuff... whilst the person in first (and quite possibly the person with the best actual racing-ability) gets banana peel after banana peel... well, it caused a lot of RAGE.
